Child’s Bill of Rights and Outline of Responsibilities
RIGHTS
-
To be
treated in a courteous and respectful manner by Centre Staff.

To
have the Centre respond to his/her needs and preferences e.g.
language, cultural, etc. to the best of Centre’s ability.

To have
the right to information about community services
-
To have
the right to refuse consent at any time in the assessment, treatment
and referral process.

To have
the right to raise concerns or recommend changes with regards to any
service without the fear of interference, coercion, discrimination
or reprisal.

To have
the right to be informed in writing of the procedure for initiating
complaints about the Centre’s policies, procedures or about a Staff
Member.

To have
the right to have his/her records kept confidential in accordance
with the law.

To have
the right to review all material in his/her file at the Centre.
RESPONSIBILITIES

To actively participate in the assessment process.
- To actively
participate in the goal setting and intervention process with the
Centre therapist(s).
- To follow
through on agreed upon actions e.g. external referrals, appointments
with other service providers.
- To be on time
for appointments and if there is a reason to cancel, to notify the
therapist in a timely fashion. If two appointments are missed
without notification, the child will be discharged.
- To inform the
Centre/Therapist if the child to be seen has been exposed to any
communicable disease e.g. chicken pox, scabies, lice, etc.
