A Social Worker is a professional who is registered with the Ontario College of Social Workers and Social Service Workers. This professional provides clinical services on an individual, family, couple/dyadic and group basis.  This provided through a variety of services aimed at helping the individual and his/her family to cope and adjust to a anew diagnosis and/or acquired trauma.  The focus of involvement is often related to the impact of crisis, disability and hospitalization on the family.  The focus may be related to other psycho-social needs or problems identified by the family such as finances, lack of support services, mental health of family members, etc.  The clinical service interventions include:

  • Crisis intervention
  • Family/Individual assessment
  • Counseling
  • Educating families and individuals re: community services and supports
  • Transitional planning
  • Bereavement/grief work/denial/adjustment
  • Coordination/community liaison/practical supports and facilitator
  • Advocacy
